As a Discharge Coordinator, you will play a pivotal role in ensuring smooth transitions for patients leaving Cornerstone's facilities.
Your primary responsibility will be to facilitate the discharge process, collaborating closely with healthcare professionals, patients, families, and external service providers to ensure continuity of care and support.

Responsibilities:
Discharge Planning:
Coordinate and oversee the discharge planning process for patients, assessing their needs and developing individualized discharge plans in collaboration with the healthcare team.
Patient Advocacy:
Advocate for patients' rights and preferences regarding their discharge plan, ensuring that their needs, concerns, and preferences are taken into account.
Resource Coordination:
Collaborate with internal and external stakeholders, including healthcare providers, social workers, community resources, and insurance companies, to arrange necessary services and resources for patients post-discharge, such as home healthcare, medical equipment, and transportation.
Patient Education:
Provide patients and their families with information regarding post-discharge care instructions, medications, follow-up appointments, and available support services to promote self-management and adherence to the discharge plan.
Documentation and Compliance:
Maintain accurate and up-to-date documentation of discharge plans, interventions, and communications in accordance with regulatory requirements and organizational policies.
Risk Assessment:
Identify potential barriers to successful discharge, such as financial constraints, transportation issues, or lack of caregiver support, and develop strategies to address these challenges effectively.
Quality Improvement:
Participate in quality improvement initiatives aimed at enhancing the discharge process, optimizing patient outcomes, and ensuring patient satisfaction.
